Job Description

Assess clients and develop individualized treatment plans to improve mobility, strength, balance, safety, and independence, including support for recovery and fall prevention. Coordinate care with occupational therapists, nurses, and personal support workers, educate clients and caregivers, maintain clinical documentation, and contribute to program evaluation and quality improvement.Job detailsCommunity Physiotherapist | Help Launch a New Home Care Program | ScarboroughWhat if your next role let you build something instead of inheriting it?A community healthcare organization in Scarborough is launching a new Community Home Care Program, and it is being built from the ground up. The physiotherapist who joins now will help decide how services are delivered, not just follow a process someone else wrote years ago.Your work will help older adults and people living with chronic and complex conditions stay mobile, safe and independent in their own homes. You will see the difference you make up close: a client walking confidently again, a caregiver who finally knows how to help, a fall that never happens.WHY THIS OPPORTUNITY IS DIFFERENT? Most physiotherapy roles hand you a set caseload in an established clinic. This one hands you a program in its first chapter. You will contribute ideas, take part in quality improvement and program evaluation, and help shape what good community care looks like here.? You will practise with genuine autonomy in clients' homes and community settings while working alongside Occupational Therapists, Registered Nurses and Personal Support Workers through shared care plans and case conferences.? Schedules are shared well in advance, usually at least a week ahead, and travel between client visits is paid working time.? The organization is deeply committed to equity, inclusion, accessibility and culturally responsive care.WHAT YOU'LL BE DOING? Helping clients regain and keep mobility, strength, balance and independence through comprehensive assessments and individualized treatment plans.? Supporting recovery after illness, injury, surgery or hospitalization across neurological, musculoskeletal, cardiovascular and age-related conditions.? Preventing falls through falls risk assessments, balance training, gait work and safe ambulation recommendations.? Easing pain through therapeutic exercise, manual therapy, mobility training and client education.? Equipping clients and caregivers with the skills to keep progress going between visits, from mobility aids and body mechanics to injury prevention and healthy aging.? Building integrated care plans with OTs, RNs and PSWs, joining case conferences and coordinating referrals to community rehabilitation services.? Keeping clear, timely and confidential clinical documentation and contributing to quality improvement as the program grows.WHAT WE'RE LOOKING FOR? A degree in Physiotherapy from an accredited university program.? Current registration in good standing with the College of Physiotherapists of Ontario.? Current CPR certification.? A valid Ontario G driver's licence and access to a reliable vehicle.? Around 3 years of physiotherapy experience is preferred, though strong candidates with less will be considered.? Experience with older adults, chronic disease and clients with complex medical needs, and with interdisciplinary teams.? Confidence with mobility assessments, balance training and fall prevention.? Comfort with electronic medical records and Microsoft Office.? Experience in community health, home care, rehabilitation, geriatrics or primary care, or in building a new program, is a strong asset.? You are a natural coach and motivator, self-directed, well organized, and excited to contribute ideas to a developing program.COMPENSATION & WORK MODEL? $42 to $49 per hour, commensurate with experience.? Standard benefits included for the full-time position.? Full-time one-year contract at 35 hours per week with a possible extension, or one of two part-time positions at 17.5 hours per week.? Work model: Community-based, delivered primarily in clients' homes and community settings.? Evening and weekend work on a rotating basis, with frequency being finalized as the program launches.LOCATION & TRAVEL? Based in Scarborough, Ontario.? Regular local travel between client homes and community locations, with travel time between visits paid as working time.HOW TO APPLY To apply or learn more, contact: bronwyn.williams@tappwfs.comABOUT TAPP WORKFORCE SOLUTIONS TAPP Workforce Solutions is a specialized recruitment and workforce solutions firm supporting organizations across skilled trades, engineering, manufacturing, and professional services.
